I think when guys go down instantly from a ball bust, it's more of a psychological reaction, a protective reflex.

I say this because the pain we feel when our balls get kicked isn't entirely instant.  There is pain right away, but it actually increases more after a couple of seconds.

So a guy who just drops is probably more just being defensive, protecting himself, even if it's an involuntary reflex.

But the guy who slowly drops, gasping for breath, eyes wide open in shock... well, that's when you know you really got him, the pain is intense, he literally can't even stand up, can't breathe, can't speak, he's totally incapacitated by the ball kick.  

But like in the video, this kind of response takes a few seconds.  It's not instantaneous.

Even with the slow reaction, there is still more pain coming too. It builds for a while. The balls hurt right away but the abdominal pain rushes in shortly after and builds when you get the balls really "good".
