Kick in the pants

So watching cnn today, you hear female anchors using a lot of metaphors like Donald trump kicked the political establishment in the pants yesterday... That metaphor has been around for decades but what does it literally mean? A kick in the ass that surprises you and gets your attention for the fight to come? Or a kick in the groin which surprises you with paralysis and can only make you stronger maybe an hour later if not longer? Got any opinions or English language historical facts people?
