I can confirm that, after orgasm, the pain is MUCH worse. In my case, I was getting squeezed during (which is awesome) but she was turned on and kept on going after. Love the term, though personally I don't like the "ruined orgasm" part of it, though I love being busted during orgasm. My favorite method is being punched, and I've had a bunch of sessions (personal and pro) where I'm getting hit continuously while masturbating. It's unquestionably the most intense orgasms I've had, and love the feeling.

Will also chime in with the +1 for additional impact post-orgasm is radically more painful - and for me mostly unenjoyable. Not in the least bit terrified as long as it is before the onset of orgasm.  The brain has difficulty processing two highly intense signals at the same time.  There is also a release of endorphins into the blood stream at that point which also helps to counteract the pain.  In this case the increased blood flow caused by the bust often intensifies the orgasm even thou sheer agony will follow immediately after the orgasm.

But if the bust occurs after the orgasm has begun (within about 3 second in order to completely ruin it) the pain is ten times more intense and it is instant off for the pleasure part and it is all pain.  Definitely very cruel.  In this case the increased blood flow makes the testicles much more sensitive than normal until the erection subsides.
